Make your wishes known

It is important that we make our wishes concerning end of life issues known to those who are close to us. By letting them know what your preferences are, you will save them a lot of unnecessary grief.

You should at least complete the Healthcare Power of Attorney form, but it’s best to do both. Let people know where the original is kept, and make copies to carry with you and to give to your doctor.

Starting on July 1, 2005, Beaufort Memorial Hospital will scan your advance directives and keep them on file in the event you need to be hospitalized. We offer this free of charge as a public service. If you want to have your advance directive scanned, take your completed forms, your insurance card, and a photo ID to the BMH Admitting department. They will scan your documents and return them. Good for your peace of mind!
